Sizes are determined by wrapping a flexible tape measure inside the bracelet and across the "gap".  If the reading on the tape measure shows 7", for example, that is the size we assign to it.   Most bracelets can be opened up maybe a half size, or closed down maybe a half size in order to get a good fit.
